N-648, Medical Certification for Disability Exceptions

The Berkshire Immigrant Center assists many people annually in the pursuit of US Citizenship. As part of this process an applicant is required to take an English, US History, and Civics test.
Some applicants are not able to learn new information because they suffer from a medical disability (like dementia or Alzheimer’s) which prohibits them from learning or retaining this information. In these cases, the only way a person can become a US Citizen is to have a doctor fill out a N648 Disability Waiver Form.
Although this form is not terribly complex, it does need to be filled out completely and in a certain way. Below we have a USCIS video link to help doctors as well as a PDF of a handout explaining the purpose and filling out of the form.
If a doctor is assisting a BIC client in the pursuit of a N648, we are always happy to advise on this form if necessary. Medical professionals currently working with our clients should email info@berkshireic.com for more information or assistance.

Parking:
Free parking is available on non-metered streets surrounding downtown and in front of our office on Allen st. for 1 to 3 hours or more as indicated by street signs. The streets near the court house (on the opposite side of the traffic circle from our office) provide up to 3 hours of free parking.
Metered parking by the hour is available on downtown streets and in city parking lots - many of which are close to our office.

Where to find us:
We are located on the 2nd floor of St. Stephen's Church, easily identifiable as the big brown church by the traffic circle in downtown Pittsfield. The Church's address is 67 East st., but our entrance is on Allen st. through the big red doors.
